Parent/guardian contact info: name, email, phone number, mailing address

Check in/Check out dates for hotel reservation

Fill this form out once for EACH guest in your party, not including your dancer.

*Guests aged 2 and under*

There is no charge for guests ages 2 and under however we do need this info for the hotel and group bookings.
